  • Overview

    Work at highest level of practice, this position will support the interdisciplinary care teams lead by a nurse practitioner to implement, monitor and evaluate patient interventions and treatment care plans. Assist with sick visit triage and clinical assessments to identify appropriate level of clinical intervention and management by nurse practitioner.

    Responsibilities

    • Provide inpatient hospital visits, coordinating and facilitate discharge planning
    • Provide in home visits within 24 – 48 hours post hospitalization to conduct medication reconciliation, ensure patient understanding of treatment plan, facilitate any services directly related to the discharge needs and engage care nurse practitioner as appropriate
    • Provide in home or telephonic triage and/or clinical assessment patients as needed and/ or assigned by nurse practitioner
    • Provide in home or telephonic targeted education to the patients, family members/caregivers as it relates to medication adherence, disease management, appropriate use of self- management condition specific tools (peak flow meter, glucose testing, insulin, nebulizers, and inhalers).
    • Communicates with all members of the internal and external interdisciplinary care team and actively participates care planning and implementation of customized treatment plans
    • Attend and participate in weekly rounding meetings with all teams to assist nurse practitioners and teams with customized treatment plans
    • Documents in Athena, PPC Inc.’s electronic medical record

    Other activities

    • Identify and develop policies and procedures that support program business objectives
    • Maintain professional growth and development through self- directed learning activities and involvement in professional, civic and community organizations
    • Maintain RN license in good standing without restrictions
    • Other duties as assigned

    Qualifications

    Required:

    • Bachelor, Associate or Diploma Degree in Nursing from accredited college
    • Active Rhode Island RN License – current and unrestricted
    • Five (5) years of applicable clinical experience
    • Previous/current experience as an RN providing clinical care in a home setting
    • High comfort level providing care in member’s home and assessing cardio pulmonary clinical status
    • Proven skills and knowledge base necessary for critical thinking and decision making
    • Strong organizational and documentation skills
    • Strong problem solving skills and attention to detail
    • Effective oral and written communication skills
    • Intermediate skills in Microsoft Office (Word, Excel, Outlook) and Electronic Medical Records (EMR)
    • Dependability – when necessary, commits to hours necessary to meet the needs of members
    • Ability to travel for work, must have access to reliable transportation. If using personal vehicle, must have valid driver’s license and proof of insurance
    • Proof of Immunization/Vaccine Records as required by Department of Health (including declinations)  

    Preferred:
