Overview

Assist the Supervisor and Case Managers to accomplish the administrative duties of Medical Management Programs. The Program Coordinator will interface with staff within the department, within the organization and with external partners, members and their families

Responsibilities

  • Complete telephonic screening of members
  • Provide information to members’ families, providers and external partners or providers, as assigned
  • Direct members and clients to internal resources
  • Documentation in Acuity Case Management software
  • Data entry of mail-in surveys and screens
  • Managing health care Incentives
  • Interface with vendors, as assigned
  • Assist in compiling and preparing reports
  • Identify opportunities for improvement and QI projects
  • Identify and pursues opportunities for professional development
  • AcuPrint: print queued letters; coordinate with mailroom; prepare resource mailings
  • AcuStrat: schedule file imports, run rules
  • Crystal Reports: letter configuration and updates; coordinate with CRT
  • Beacon collaboration: run additional Cognos IRE member specific reports; send reports to Beacon as needed; generate Beacon referrals; follow-up on referral outcomes
  • Peer Navigator collaboration: generate referrals; input referral outcomes; scan and attach encounter data
  • Other duties as assigned
  • Corporate Compliance Responsibility - As an essential function, responsible for complying with Neighborhood’s Corporate Compliance Program, Standards of Business Conduct, applicable contracts, laws, rules and regulations, policies and procedures as it applies to individual job duties, the department, and  the Company. This position must exercise due diligence to prevent, detect and report unlawful and/or unethical conduct by fellow co-workers, professional affiliates and/or agents

Qualifications

Required:

  • Associates Degree or equivalent years of background and experience to equal degree
  • One (1) to Three (3) years in a related health care or social service field
  • Intermediate to Advanced skills in Microsoft Office (Project, Word, Excel, PowerPoint, Outlook), internet
  • Strong interpersonal skills
  • Strong organizational and prioritization skills
  • Strong clerical skills and attention to detail
  • Strong data entry skills
  • Strong time management skills
  • Ability to problem solve and sound judgment
  • Ability to work independently and as part of a team
  • Ability to keep accurate records and prepare reports
  • Ability to work with minimal supervision
  • Strong oral and written communication skills including excellent customer service skills
  • Demonstrated ability in prioritizing and completing multiple assignments and meeting deadlines
  • Knowledge of medical terminology

Preferred:

  • Experience with Case Management Software or Electronic Health Records
  • Clinical experience
  • One (1) to Three (3) years in an HMO environment
